Anxiety is a common issue among pre-operative patients, often stemming from uncertainty about the procedure, fear of pain, and concerns about the surgerys outcome. While anxiety management is a nursing concern, many patients still experience high levels of anxiety, impacting their physical, psychological, and post-operative recovery. This study aimed to analyze the effect of spiritual support, specifically prayer guidance, on anxiety in pre-operative patients at the Central Surgical Installation of Kanjuruhan Regional General Hospital. The research design was a pre-experimental, one-group pre-test and post-test approach. The results showed that most pre-operative patients experienced moderate anxiety (52.5%) before the intervention, with a median score of 64. After the spiritual support intervention, the majority of respondents experienced a decrease in anxiety to mild (52.5%), with a median score of 47. The Wilcoxon test revealed a significant influence of prayer guidance on anxiety, with 76 out of 80 respondents experiencing a decrease in anxiety levels. Pre-operative patients benefit from spiritual support, such as prayer guidance, which aids relaxation, strengthens patient confidence, and provides courage through spiritual connections, ultimately reducing anxiety levels.

Spiritual support in the form of prayer guidance is recommended to be integrated into routine nursing practice to reduce anxiety in pre-operative patients.Nurses should receive specific training to improve the quality of interventions.Patients are expected to actively participate in prayer guidance and be open to emotional support from healthcare workers.Prayer guidance not only facilitates relaxation responses but also strengthens the patients confidence and courage through a deep spiritual connection.This practice is a simple, inexpensive, and applicable approach in various health facilities.Hospitals should adopt spiritual support programs by providing dedicated spaces and clear SOPs.Further research with stronger methods is recommended to strengthen the evidence of this interventions effectiveness and expand its application to different patient populations.

To further enhance patient care, future research could explore the combination of spiritual support with other interventions to produce an even more optimal effect in reducing patient anxiety. Additionally, investigating the effectiveness of this approach in diverse patient populations and developing practical guidelines for its implementation in healthcare systems would be beneficial. Finally, conducting studies with stronger methods to strengthen the evidence of this interventions effectiveness is recommended.
