Sciatica is a radiating lower back pain problem that arises due to stimulation of sensory fibers originating from the posterior lumbar 4th to sacral 3rd roots and this can occur in any part of the isodicus nerve (L4-S3) before reaching the back surface of the leg containing nociceptive (pain) receptors, which are stimulated by various local stimuli such as mechanical thermal and chemical stimuli will be responded to by the release of various inflammatory mediators that will cause the perception of pain. The role of physiotherapy in this case includes reducing back pain, increasing the limited range of motion of the trunk joints, increasing muscle strength and reducing spasms in the muscles concerned. The therapy given to resolve this problem is by using Microwave Diathermy (MWD), Transcutaneous Electrical Nerve Stimulation (TENS) and William Flexion Exercise. This research is a case study, raising patient cases and collecting data through the physiotherapy process. The modalities provided are Microwave Diathermy (MWD), Transcutaneous Electrical Nerve Stimulation (TENS) and William Flexion Exercise. The results of this study indicate that physiotherapy management using Microwave Diathermy (MWD), Transcutaneous Nerve Stimulation (TENS), and William Flexion Exercise for this trunk movement was carried out according to the SOP, resulting in reduced pain, increased range of motion, increased muscle strength, decreased muscle spasms, and increased functional activity. The conclusion of this study is that Microwave Diathermy, Transcutaneous Electrical Nerve Stimulation, and William Flexion Exercise can reduce pain, increase range of motion, decrease spasms, increase muscle strength, and improve the patients functional activity. Successful therapy also requires collaboration between the patient and the physiotherapist.

Penelitian ini menyimpulkan bahwa intervensi fisioterapi menggunakan modalitas microwave diathermy (MWD), transcutaneous electrical nerve stimulation (TENS), dan William flexion exercise efektif dalam menurunkan nyeri dan meningkatkan kemampuan aktivitas fungsional pasien dengan low back pain e.Hasil terapi menunjukkan penurunan nyeri, peningkatan fleksibilitas dan kekuatan otot, serta peningkatan aktivitas fungsional pasien.Keberhasilan terapi juga membutuhkan kerja sama antara pasien dan fisioterapis.

Berdasarkan hasil penelitian ini, terdapat beberapa saran untuk penelitian lanjutan. Pertama, penelitian lebih lanjut dapat dilakukan dengan melibatkan jumlah partisipan yang lebih besar dan menggunakan desain penelitian yang lebih kuat, seperti randomized controlled trial, untuk memperkuat bukti efektivitas kombinasi terapi MWD, TENS, dan William Flexion Exercise. Kedua, penelitian dapat difokuskan pada identifikasi faktor-faktor spesifik yang memprediksi respons pasien terhadap terapi, seperti tingkat keparahan nyeri awal, durasi gejala, atau karakteristik individu pasien. Hal ini akan memungkinkan pengembangan protokol terapi yang lebih personal dan optimal. Ketiga, penelitian lanjutan dapat mengeksplorasi efektivitas jangka panjang dari intervensi ini, serta menginvestigasi potensi penambahan modalitas terapi lain, seperti latihan penguatan inti atau terapi manual, untuk meningkatkan hasil klinis dan mencegah kekambuhan nyeri punggung bawah.
