Pregnancy causes a lot of changes and adjustments in women that cause discomfort. Discomfort is an unpleasant feeling for the physical or mental condition of pregnant women. Discomfort that appears in pregnant women in the third trimester is an increase in the frequency of urination, constipation, oedema, lower abdominal pain, haemorrhoids, and varicose veins. Oedema in the lower limbs is caused because blood flow is inhibited, so nonpharmacological therapy is carried out with ankle pumps and leg elevation positions. This therapy is beneficial for controlling swelling and can help pump blood to the heart through muscle contractions. The purpose of the study was to evaluate how ankle pump therapy and leg elevation position can overcome oedema in the lower limbs in pregnant women in the third trimester. This study is a case study with a comprehensive obstetric care approach to Mrs. M and data collection, observation and management of ankle pump therapy and leg elevation position for 1 (one) week routinely 2-3 times a day with a duration of 5-10 minutes. The results obtained from the discomfort care intervention in the third trimester that the oedema in the lower limbs has been reduced so that it feels comfort. In conclusion, the effectivity of ankle pump and leg elevation position can reduce oedema on the leg of pregnant women in the third trimester.

The study concluded that the combination of ankle pump exercises and leg elevation is effective in reducing lower limb oedema in a third-trimester pregnant woman.This non-pharmacological intervention, implemented consistently over one week, resulted in a noticeable reduction in oedema and improved the patients comfort.Therefore, this approach can be utilized to manage oedema and enhance well-being during the third trimester of pregnancy.

Berdasarkan hasil penelitian ini, beberapa saran penelitian lanjutan dapat diajukan untuk memperdalam pemahaman mengenai efektivitas terapi kombinasi ankle pump dan elevasi kaki pada ibu hamil. Pertama, penelitian dengan desain studi yang lebih luas, seperti randomized controlled trial (RCT), perlu dilakukan untuk menguji efektivitas terapi ini pada populasi yang lebih besar dan beragam, sehingga hasilnya dapat digeneralisasikan. Kedua, penelitian lebih lanjut dapat mengeksplorasi pengaruh terapi kombinasi ini terhadap parameter fisiologis lainnya, seperti tekanan darah, volume cairan, dan sirkulasi darah, untuk memahami mekanisme kerjanya secara lebih komprehensif. Ketiga, studi kualitatif dapat dilakukan untuk menggali pengalaman dan persepsi ibu hamil terhadap terapi ini, termasuk faktor-faktor yang memengaruhi kepatuhan dan keberhasilan terapi, sehingga intervensi dapat disesuaikan dengan kebutuhan individu dan meningkatkan kepuasan pasien. Penelitian-penelitian ini diharapkan dapat memberikan bukti ilmiah yang lebih kuat untuk mendukung penerapan terapi kombinasi ankle pump dan elevasi kaki sebagai bagian dari perawatan komprehensif pada ibu hamil dengan oedema.
