Hydatidiform mole is a form of gestational trophoblastic disease (GTD) characterized by abnormal proliferation of trophoblastic tissue. It commonly presents with excessive uterine enlargement in early pregnancy and markedly elevated β-hCG levels. This report describes a case of complete hydatidiform mole in a 35-year-old woman, in which diagnosis was primarily established through characteristic imaging findings on pelvic ultrasonography. The patient underwent methotrexate chemotherapy, and clinical outcomes were monitored. The main outcomes included a favorable response to methotrexate therapy, evidenced by a decline in β-hCG levels and improvement of ultrasonographic findings. Cutaneous adverse effects were observed during treatment, manifesting as mucositis, papulovesicular lesions, melanonychia, and anagen effluvium.

Pelvic ultrasonography plays a crucial role in the diagnosis and follow-up of hydatidiform mole.Careful monitoring of methotrexate-related adverse effects, particularly cutaneous manifestations, is essential for patient safety.This case highlights the importance of integrating imaging, laboratory evaluation, and toxicity surveillance in the routine management of gestational trophoblastic disease.

Penelitian lebih lanjut diperlukan untuk mengidentifikasi biomarker yang dapat memprediksi respons terhadap kemoterapi methotrexate pada pasien dengan mola hidatidosa, sehingga memungkinkan personalisasi terapi dan meminimalkan efek samping. Studi prospektif yang melibatkan kohort pasien yang lebih besar perlu dilakukan untuk mengevaluasi efektivitas intervensi pencegahan mucositis, seperti penggunaan agen pelindung mukosa atau protokol perawatan mulut yang intensif, dalam mengurangi morbiditas terkait kemoterapi. Selain itu, penelitian lebih lanjut diperlukan untuk memahami mekanisme yang mendasari manifestasi kulit akibat methotrexate, termasuk peran faktor genetik dan imunologi, untuk mengembangkan strategi terapeutik yang lebih efektif dan mengurangi dampak psikologis pada pasien.
