The unpreparedness of pregnant women in facing childbirth is one of the factors causing the maternal mortality rate. Supporting efforts to improve the quality of maternal health service to reduce maternal mortality rate, the government is empowering families with a program for ownership and utilization of KIA books. The KIA book details for birth preparedness including estimated birth date, birth companion, place and assistant of delivery, savings and emergency cost for childbirth, vehicle preparation, blood donation, mother and baby equipment, important document, and plans to contraception method. Birth preparedness for pregnant women in the third trimester based on the KIA book is still low. The main aim of the study was to compare the description of birth preparedness for pregnant women in the third trimester based on the KIA book at Puskesmas Mentok, 2025. The type is research a quantitative descriptive observational design with a cross sectional study. The population consist of all pregnant women in the third trimester to visiting Puskesmas Mentok in March 2025 period, amounting to 50 respondents. Sampling was caried out using the total sampling technique. There are still around (8%) of pregnant women in the third trimester who are birth preparedness in a less than good approach. The risk of complicatons during pregnancy and childbirth is still high, one of which is when a pregnant woman experiences bleeding and requires special assistance in the form of a blood transfusion, but the pregnant woman does not yet have a candidate blood donation to prepare of emergencies.

The study found that most pregnant women in the third trimester were aged 20-35 years, had a low to moderate education level, and were not employed.The majority of respondents had already planned for the delivery process, including knowing the estimated date of delivery and having a birth companion.However, a significant proportion had not yet prepared a blood donor or posted a P4K sticker at their homes.Therefore, it is crucial to enhance preparation for childbirth among pregnant women, particularly regarding blood donor preparedness and P4K sticker placement, to reduce maternal mortality risks.

Berdasarkan hasil penelitian ini, beberapa saran penelitian lanjutan dapat diajukan. Pertama, penelitian lebih lanjut perlu dilakukan untuk menggali lebih dalam faktor-faktor yang mempengaruhi rendahnya kesiapan ibu hamil dalam mempersiapkan calon pendonor darah, termasuk persepsi, pengetahuan, dan akses terhadap informasi mengenai donor darah. Kedua, studi kualitatif dapat dilakukan untuk memahami pengalaman dan tantangan yang dihadapi ibu hamil dalam mempersiapkan persalinan, khususnya terkait dengan keterbatasan finansial, dukungan sosial, dan akses terhadap fasilitas kesehatan. Ketiga, penelitian intervensi perlu dirancang dan diimplementasikan untuk meningkatkan kesadaran dan perilaku ibu hamil dalam mempersiapkan persalinan yang aman dan sehat, dengan fokus pada penyediaan informasi yang komprehensif, pelatihan keterampilan, dan pendampingan oleh tenaga kesehatan. Penelitian-penelitian ini diharapkan dapat memberikan kontribusi signifikan dalam upaya menurunkan angka kematian ibu dan meningkatkan kualitas pelayanan kesehatan maternal di Indonesia.
