Introduction: Traumatic brain injury (TBI) represents a major global health burden, because it is the foremost causes of death, particularly in young adults. Elevated IL-6 and IL-10 serum levels in severe TBI have been identified as an early indicator of unfavorable clinical outcomes. Pegagan (Centella asiatica (L.)) has antioxidant, neuroprotective, neurogenerative, anti-inflammatory properties. Our goal of this study was to find out what happened to IL-6 and IL-10 levels in blood of the TBI rat model when they were given pegagan extract (Centella asiatica (L.)). Subject and Method: experimental research design. For 3 days, there were 4 groups of male Wistar strain rats (Rattus norvegicus) subjects, consisting of three treatment groups (pegagan extract doses of 300 mg/kgBW, 500 mg/kgBW, and 1000 mg/kgBW) and one control group. On the 4th day, blood was taken, and we continued to examine IL-6 and IL-10 serum levels by ELISA. Result: A dose-dependent and significant increase of IL-6 and IL-10 serum levels was observed in the treatment groups compared with control group.

The study demonstrated that administering pegagan (Centella asiatica (L.)) extract at doses of 300mg/kgBW, 500mg/kgBW, and 1000mg/kgBW significantly increased IL-6 and IL-10 serum levels in a rat model of traumatic brain injury.This increase may be attributed to heightened macrophage activity induced by the natural compounds within pegagan and the subsequent neuroprotective effects within the central nervous system.The findings suggest a complex interplay between inflammation and neuroprotection following TBI, warranting further investigation.

Penelitian lebih lanjut perlu dilakukan untuk menginvestigasi mekanisme molekuler spesifik yang mendasari peningkatan kadar IL-6 dan IL-10 setelah pemberian ekstrak pegagan, termasuk jalur pensinyalan yang terlibat dan efeknya pada sel-sel glial dan neuron. Selain itu, studi prospektif pada model hewan yang lebih kompleks, seperti yang meniru cedera otak traumatis ringan hingga berat, diperlukan untuk menentukan apakah efek yang diamati dapat diterjemahkan ke dalam pengaturan klinis. Terakhir, penelitian yang mengeksplorasi potensi kombinasi ekstrak pegagan dengan terapi lain, seperti rehabilitasi atau farmakoterapi, dapat memberikan wawasan tentang pendekatan pengobatan yang lebih komprehensif untuk cedera otak traumatis, dengan mempertimbangkan variasi dosis dan durasi pemberian untuk mengoptimalkan hasil terapeutik dan meminimalkan efek samping.
